As a news report in the media reveals, a 52-year-old member of staff was working an evening shift at the plant in Shepton Mallet when the incident happened, around a year ago. The employee went to investigate the cause of a stoppage in a machine which takes empty bottles of a conveyor to be filled, although he was covering for a colleague and did not usually work in the respective station.

The employee put his hand into the machine which automatically restarted catching his clothing and pulling his right arm into the moving parts. An investigation from the Health and Safety Executive (HSE) found that Brothers had made modifications to the access to the machine before the accident but had not installed a device or guard to prevent workers accessing the machine while it was still active.
